First Visit
A parent or legal guardian must accompany your child for the first visit. If a legal guardian rather than a parent accompanies your child, the guardian must bring guardianship and custody papers for your child to be seen.

Our practice has been awarded Patient-Centered Medical Home (PCMH) Level 3 Recognition by the National Committee for Quality Assurance (NCQA). As part of our medical home, your child's primary care provider leads a care team dedicated to addressing your childs comprehensive health care needs arranging care with other qualified professionals as needed. The care team may include our doctors and nurses, care coordinators, psychologists, social workers and/or lactation consultants.
