Outpatient Radiology Services
Complete the Radiology Order Form and fax it to (407) 567-5903. Medical imaging appointments cannot be made until insurance authorization has been received. Processing can take up to 14 business days. Once authorization is received, our PSS team will contact the family to schedule an appointment.
